This is a simple guide to completing our online forms when a loved one passes away. We request that these forms be filled as soon as possible after you receive the link.

Preparing to complete the online questions.

1. Give yourself some time. Filling out the forms may take up to half an hour to complete.

2. Complete the questions on a computer or tablet. You can use a mobile phone, however it is easier to complete on a computer or tablet. 

3. It is best to complete the forms in one sitting. Please have the following personal information about the deceased ready with you:

  • Full name
  • Dates of birth and death
  • Residential address, address of place of death
  • Place of birth
  • Year arrived in Australia if born overseas
  • Parents names and dates of birth
  • Children’s names and dates of birth
  • Names of spouse/domestic partner
  • Details of any marriages – city, date, name of who married
  • Occupation

You will also need to fill in personal details about yourself, including:

  • Full name
  • Dates of birth 
  • Residential address
  • Contact number and email address
  • Occupation details
  • Relationship to the deceased
  • Secondary contact details if we cannot reach you

4. If you live in NSW or WA please have a photo ID ready and available.

5. Do not use an "incognito" or "private" mode on your browser and do not have any active pop up blockers running when completing the online forms.

Completing the online questions.

1. All questions must be answered. If there are any questions you do not have an answer for, please write ‘Unknown”. We can rectify this later if you learn the information and adjust accordingly if needed.

2. When you complete the questions, you will be able to preview your answers before submitting. Make sure to read through all your answers carefully before proceeding. 

3. You will know you have successfully submitted the questions when you see the thank you screen. You will also receive an email thanking you for completing the online forms.

4. If you do not successfully submit please contact us for assistance. Call your arranger on 1800 071 176. We may be able to retrieve the information in our system.
